Ultra Mushroom is one of the best HP-replenishing items that you can find in Paper Mario: The Thousand-Year Door. Because it restores a high amount of HP, they're much rarer than their Super Mushroom counterpart and cost well over 100 Coins. However, the latter half of Thousand-Year Door features stronger enemies and bosses, and while you can recover HP with the Special moves Sweet Treat/Sweet Feast, you'll likely want to save your Star Power in case you find yourself in a pinch. Depending on how you're leveling up Mario, an Ultra Mushroom can fully restore his or his ally's HP, making it a worthy item to add to your inventory.

How Much Do Ultra Mushrooms Cost?
Ultra Mushrooms can be found and picked up in the latter areas, such as X-Naut Fortress and the Palace of Shadow. If you've already cleared out both areas, then you're going to need to buy them. Ultra Mushrooms are sold for 200 Coins at the Deepdown Depot in Rogueport Underground. You can also get them from Charlieton in Rogueport Plaza for 120 Coins, or in Pit of 100 Trials (prices increase every 10th floor).
You can reach Rogueport Underground by entering the grate in Rogueport West, and jumping across the platforms to the other side. Deepdown Depot is the store on the right behind the plaza's fountain. At 200 Coins, you probably won't have enough at the start of your playthrough.
Once you have Bobbery, you can blow up the collapsed column in front of Merluvlee's House to open another path to Rogueport Underground.
Charlieton (Rogueport Plaza location) sells Ultra Mushroom for the lowest price you'll find in-game. At 120 Coins, it's one of his rare good deals, so you'll want to buy one whenever he has one in stock. You can change Charlieton's wares by leaving the area (two screen transitions) and returning. Keep doing this until he has the item you want.
You can also get an Ultra Mushroom by completing the "Help me restock!" Trouble. This Trouble will unlock at the Trouble Center after you've cleared Chapter 1. It requires you to give Plenn T. (At Toad Bros. Bazaar in Rogueport) five Courage Shells.

You may have noticed that aside from Mushroom Steak, all Ultra Mushroom recipes require you to use two ingredients. Zess T. In Rogueport (next door to Toad Bros. Bazaar in the plaza) will cook whatever item you hand her, but if you find the Cookbook key item (locked inside a chest in the hidden room on the first hallway/floor of Creepy Steeple) and complete her "Seeking legendary book!" Trouble (unlocked after Chapter 4), you'll be able to cook with two ingredients instead of one.
The most recommended meal you should cook with the Ultra Mushroom is the Jelly Ultra. This requires a Jammin' Jelly, which can be bought from the same stores as the Ultra Mushroom, and for the same price. Otherwise, Maple Ultra is a good substitute. If you can't find Ultra Mushrooms but have a Jammin' Jelly and Point Swap, then you can have Zess T. Combine them to get an Ultra Mushroom.
