Marineford and Impel Down destroyed Luffy's body on another level, and all that is to say that his body has taken a beating over the years.

Luffy's Life Is Running Short
After Luffy overused Gear 2 Vs Lucci, he himself knew that he was shortening his lifespan using the technique. When Luffy used Gear 2 and 3 at the same time against Moria, the Straw Hats were worried that he was shortening his life by doing so. Similarly, Luffy lost 5 years of his life to be cured from Magellan's poison, and then he took more shots from Ivankov during the war, boosting his powers more at the cost of his life.

Although Luffy has learned to control Gear 2,3, and 4 to a point where he doesn't overexert himself anymore, Gear 5 is a different story altogether. Luffy fought Kaido in Wano, and when he forced himself to re-enter Gear 5, Kaido clearly stated that if he pushed himself too much, he would die. To this, Luffy responded by saying that they'd all become bones anyway.

Will Luffy Die In One Piece?
Clearly, Oda has established a plot point where Luffy could potentially die in One Piece due to his worsening physical condition. But will Luffy really die? The answer is likely no. Luffy dying would be a major failure for Chopper, who wants to cure all diseases in the world. If Luffy ever does get sick, which he likely will, it will be Chopper's time to shine and save his life.
