After completing a few Nioh 3 Myths in Warring States, players will be able to tackle the Headaches Myth accepted from Tokugawa Ieyasu. While talking to him at the top of Hamamatsu Castle, he will mention that he needs medicine because he is dealing with awful headaches and wants the player to check out the village on Futamata Road. Once you make your way to the village, deal with any enemies you come across nearby and head inside one of the houses to find a corpse you can loot. Search the body to get Craft Guide: Ghostly Call. This isn't the medicine itself but only the recipe for it, so you can now head back to Hamamatsu Castle.

At the castle, go to the top and talk to Hattori Hanzo, who will be by Ieyasu's side. He will mention that the headaches are still there, and some Yokai have appeared inside the castle.

After this conversation, head down to find Nuppeppo at the gate of the Castle. Once it spots you, it will start rolling away, and for now, all you have to do is chase it until it reaches a dead end. At the far side of the Castle, upon reaching a wall, it will now become hostile towards you, and a boss fight will begin.

You can fight the boss using either style, depending on how many skill points and hours you have put into either one of them. Most of the boss's attacks are fairly easy to deflect, so the Samurai style can make light work of this boss. Once the boss has been dealt with, it will drop the Yokai Gallstone.

Take this Gallstone to the Rift and go to the Blacksmith. Here, you will have the option to Forge Tools, and a new recipe will be added for you to craft. Craft the Ghostly Call and head back to the Hamamatsu Castle and give the medicine to complete this quest.
