Together with Janet Cage and the pink ninja Floyd, Madam Bo is one of only three new characters introduced in Mortal Kombat 1. She started life as a story character who helped train Raiden and Kung Lao in Fengjian Village, but later became a surprise Kameo fighter as part of the game's second Kombat Pack.

To perform Madam Bo's Head Busser Fatality, players will need to press Back, Forward, Down, Kameo (R1 on PlayStation, RB on Xbox, or R on Switch) while standing at medium range. If done correctly, Madam Bo will kick her opponent in the chest, take a drag of her cigarette, and then kick the other kombatant's head clean off. The head flies up into the air along with the items on the tray that she's holding, but Madam Bo is able to catch them as they come back down. The head lands in one of the bowls and the body that it was once attached to falls lifelessly to the ground, all while Madam Bo takes another drag of her cigarette and strikes a victory pose.
