In the build-up to the game's release, it quickly became clear that Mafia: The Old Country is not the long-awaited Mafia 3 sequel that many franchise fans had been hoping for. It's actually a prequel, with the game's story taking place in early twentieth-century Sicily, a few decades before the events of the very first Mafia game.
All Mafia: The Old Country Voice Actors (& Where You Know Them From)
A complete Mafia: The Old Country cast list for those struggling to put names to some of the main faces and voices found throughout the 2025 title.
As series fans will already know, there are numerous Mafia characters with Sicilian heritage, so it should come as no surprise to learn that there are several returning characters in Mafia: The Old Country. Some only make small cameos, but there are one or two who play a significant role in the series' latest outing.

All Returning Characters in Mafia: The Old Country
There are four returning characters in Mafia: The Old Country, the most notable of which being Leo Galante, who fans will know from the series' second and third mainline installments. Then there's Giuseppe Palminteri, another Mafia 2 character who plays a fairly significant role in the events of Mafia: The Old Country. The last two returning characters are Frank Vinci and Sam Trapani. These two only make minor cameos, but it's nice to see them again nonetheless.
Leo Galante
Most Mafia fans know Leo Galante as the Consigliere for the Vinci Crime Family, although in Mafia: The Old Country, they get to see him in a very different light. He and his grandfather Niccolo control Porto Almaro and work closely with the Torrisi Crime Family, which sees him serve as a mentor of sorts to the game's main protagonist, Enzo.
Giuseppe Palminteri
When the Torrisi family needs a forger, it makes sense that they would turn to Giuseppe Palminteri. As Mafia 2 players already know, Giuseppe goes on to become Empire Bay's resident fixer, providing Vito with all manner of illicit materials, from forgeries to firearms. With this in mind, it's nice to see him sharing his expertise with Enzo some half a century earlier.
Frank Vinci & Sam Trapani
Sam serves as one of the caporegimes for the Salieri Crime Family in Mafia: The Definitive Edition, while Frank is the head of the Vinci Crime Family in Mafia 2. Both play bit-part roles in Mafia: The Old Country, though, with their appearances serving more as fan service than anything else.
