To get the Deadly Poison Perfume Bottle in Shadow of the Erdtree, you must head to the Pillar Path Cross Site of Grace located southeast of Castle Ensis. From the Castle Front Site of Grace, summon your Torrent and start riding it in the southeast direction. You will come across multiple patrolling Messmer Soldiers who you can either deal with or ignore as you are on horseback.
After passing the wooden blockade, take the left path when the paths split. Soon, you will reach a Site of Grace, and next to it is a 
Scadutree Fragment and Thiollier, which is selling the Deadly Poison Perfume Bottle.
