There are a total of two different Fisher Family Documents. One of these is located inside the Town Hall, and the second is in the Asylum building in the basement. You need to be in or after the story mission called Cries Trapped In The Walls in order to get into the Asylum.

The last name is spelled both Fisher and Fischer in Dying Light: The Beast. It is not clear which is correct or why there are multiple spellings, but if you find something with the last name Fisher or Fischer, it will fit into this category.

All six of the patient files you need to complete this trophy are located in the Asylum. This is a higher-level area, so you won't be able to get to this one until you are mostly done with the story. You need to start the mission called Cries Trapped In The Walls, which is the third from the last story mission in the game. Once inside the Asylum, you will be able to nab all six of the patient files while completing this quest chain.

Olivia has three locations where she has left recordings or had recordings left for her. Two are inside the Monastery, one is in the Industrial Park, and the last is in the western-most Power Plant. If you cannot access these recordings, it may be because you have not progressed the story far enough.
