Diablo 4: Vessel of Hatred picks up the base game's story and sends players on a quest to save Neyrelle from the ever-creeping clutches of Mephisto. The narrative alone is worth experiencing, but if you'd much rather concentrate on the gameplay side of things, you'll still need to finish the story in order to get to the meat of the game.
The main campaign for Diablo 4: Vessel of Hatred is fairly long, and players can expect to sink a bunch of hours into it if they want to start grinding the endgame. If you're not quite sure where you are in terms of story progression, here's the full list of main quests that you need to do in this expansion.

Almost every quest here will have you following quest markers and doing whatever the objectives list tells you to do. However, some do involve puzzle solving, and most key expansion features, like the new Mercenaries and Reinforcements system in Diablo 4, are unlocked by going through the campaign.
Once you finish the main story, you'll be given the option to skip or replay Vessel of Hatred with a new character. Additionally, Helltides and seasonal events will be playable even if you haven't finished the campaign, though Nightmare Dungeons, The Pit of Artificers, Infernal Hordes, and the new Kurast Undercity activities will still be level-gated or locked behind quest progression.
The Dark Citadel, Diablo 4: Vessel of Hatred's co-op dungeon, only becomes available after all story modes are completed. It is locked behind the Crater of Lost Souls quest, which players can start once they finish Deeds of a Champion. At this point, all new features of the expansion should be available to players.
