There aren't many NPC side quests in Deathbound, at least not to our knowledge. But this Soulslike does have one pretty early on in the second area of the game, but it's relatively easy to miss. While Rafaili does speak to you as you walk by where he's staying in the Lower Districts, you don't have to stop and talk to him (he'll only talk to Iulia anyway), and he only goes into the specifics of what he wants if you talk to him multiple times.
Elden Ring: Rare Items That Are Easy To Miss
With how many items there are in Elden Ring and its DLC, players are bound to miss out on some of the rare things in FromSoftware's masterpiece.
That said, once you do, he'll give you a bit of an errand to run for him, with a nice reward if you happen to find and enter his home. So, let's go a bit more into what this side quest entails in Deathbound, and where to go for it.
Illion's Toy Location
Near the Door of the Old Ones in the Lower District
Rafaili and his son Illion are some of the first NPCs you encounter in Deathbound in the Lower Districts of Akraytia. Of course, like a lot of From Software games, you primarily talk to them through their doors, only seeing them at the end of their questline. Iulia Tchevlaskia seems to know Rafaili personally and knows him to be a very kind individual, which is exactly why he's so worried about his son Illion and his missing toy. This is exactly what Rafaili asks you to find, saying that they left it in their house in the lower districts which can be found near some 'Technology of the Old Ones'.
The technology Rafaili is talking about near his home being near is the giant square door you open in the second half of this area that leads back to the second Phylactery you encounter in the Lower Districts. By this point you should have already found and opened this shortcut, so simply head back to this door and you'll see Rafaili's dwelling right next to it, as it is distinct from the other ones due to its wooden door. Head inside and grab Illion's Toy as well as the Sayabakn Turban Artefact.

After grabbing the toy here, you can head back to where you first spoke to Rafaili, only to see that his door is now open. With the Essencemancer monsters and the Death Cult swarming the streets of Akraytia, an open door here is very foreboding (as it usually tends to be in a Soulslike), and when you head inside you'll see exactly why. Heading out onto Rafili's patio you'll see what has (sadly) become of the father and son and Iulia is, justifiably so, very disturbed at this scene.
After her oddly brief despair, you can pick up the items underneath their pyres to get 1 Purest Essence (this is actually a bit of lore) and a Large Crystallised Essence. As you can probably tell, the Purest Essence is likely from Illion while the Large Crystallised Essence is from Rafaili, and these two Consumables might seem like bad rewards for this sidequest, but the Sayabakn Turban was the actual reward all along, as this Artefact is pretty amazing.
