The romance Denji experiences in life is more of a survival horror, rather than a romance. Although she is certainly the most significant woman in the life of Denji, along with Makima, Power takes the last place in the list of romantic interests for one simple reason: they are brothers and sisters in all respects. After the International Assassins arc ordeal, the two were allowed to have an intimacy, but Denji was far above it, and as he was able to emotionally connect with Power without it being romance.
They would share the bathing and even sleep together, not as a result of lust, but because they were two damaged children who feared the dark. Denji himself openly understands that he loves Power, but with no romantic feelings at all, and that their spark is not physical. It is the only relationship that Denji was selfless about; a platonic love that has defined his new vision of what it means to love someone without payback.

Makima
Makima is the epitome of toxic manipulation. To her, every human is just a means to an end. She was never in love with Denji, but with the Chainsaw Man within him. Denji was just a leash to her to hold on to her idol. She exploited his touch-starved instinct, assured him a normal life, and favors just to methodically ruin all that he had ever loved. Her romance was a masterpiece in grooming and mental torture, intended to destroy his spirit so that she could own the Hero of Hell. She is the weakest bond Denji has ever had, as it was founded on a lie — a parasitic bondage where he was under the impression he was a dog as opposed to a human being.
Himeno
Himeno is a complex character in the life of Denji. Being an experienced hunter who had lost too many partners, she was in need of a relationship rather than the need to attend another funeral. Her "interest" in Denji was fairly unethical, considering the age difference and her actual love interest, Aki, was too blinded by revenge to ever notice her.
But despite the red flags, she was among the very few people who treated Denji like a human being. She did not wish his heart as a mission, but as a companion to aid her in forgetting the "Easy Revenge" which had eaten her life. Had they been of equal age, she could have been the only woman ever to show him a steady, civilian sort of love, though it was perhaps followed by a surplus of cigarettes and mutual trauma, but hey, that's part of the charm of being in love.

Kobeni Higashiyama
The fact that Kobeni is included as a love interest is nearly accidental, at the behest of the Pochita version of Chainsaw Man. The date they went out was a nightmare scenario that was surreal as she was coerced into taking part in a mock-up of an ordinary outing in a state of fright over her life.

To be honest, Kobeni is a literal reflection of Denji, but in an abstract way. No one actually comprehends the plight of being a victim of the devilish world, as she is the only one who knows what it is like to be a chess piece. Although the romantic chemistry was never present, their time spent together in the burger shop was a rare case of two individuals being in their mutual misery, not having to go through any grand schemes as Denji is used to doing when interacting with women.
Fumiko Mifune
Fumiko is another type of horror in the life of Denji: the obsessive admirer. She is a devout follower of the special division of Public Safety and alleges that she is a huge fan of Chainsaw Man to the extent of defending Denji in her own twisted manner. Her interest is not stronger than that of Makima except in the fact that she appears to like the mayhem that Denji causes, but it is superficial. She is drawn to the "hero" and the "legend" and often disregards the real teenage boy who is struggling to support a pack of dogs and a little sister. She serves as a reminder that, despite Denji being liked, it is not usually for the right reasons.

Asa Mitaka
It is with Asa Mitaka that Denji has finally encountered something that can be considered more of a real high school romance, no matter a mess it is. They are both outcasts in society, and they are not aware of how to operate in a normal society. Their date at the aquarium was a social anxiety and awkwardness nightmare, but it was real, and their connection felt genuine.
Asa did not come to Denji like Makima or Reze, as a trained agent or a manipulator first, but merely as a lonely girl seeking someone to relate with. Their relationship is founded on the mutual energy of loners, which is why it is one of the most relatable and most powerful romantic possibilities in the series, since it is founded on two people grappling with how to be human together.
Yoru
The way Yoru is interested in Denji is inseparable from the interest in Asa, but it has its own weight. Being the War Devil, her task is to kill him, but being connected to a brain by Asa, she experiences the same heartbeat fluttering and real attraction Asa does. This makes it an interesting and high-stakes game in which the interest involved is literally the battle between the desire to destroy and the desire to love. The emotions of Yoru are strong since they are beyond her control; she is a Horseman of the Apocalypse and is corrupted by the most human emotions of the girl she inhabits, and their relationship is the strongest and most dangerous in the manga.
Reze
Reze is the most impactful love interest in the life of Denji since she was the first to provide him with an option. Though her first intention was a Soviet Union mission, their time together, the night swim, the café lessons, and the laughs shared, was the first time Denji experienced a spark that was not about his heart of Chainsaw. Reze is a weapon who was a child soldier and never experienced a typical life, and in Denj,i she saw someone like her.

In fact, Reze fell in love with him, unlike Makima. The final evidence of her sentiments was her decision to return to the café from the train station at the end of the arc, knowing very well what she was going to face. She still chose Denji instead of her mission or even her life, and the tragedy of their lost connection is the most touching situation in the series. She was the one who indeed fell the hardest, and it proved that it is still possible to find a true heartbeat towards another, even in a place of survival horror.
